Using Information within SD Health Link to Improve Outcomes of Care

Assistance with Community Health Assessment

This request came from a local health department seeking data on the number of cases with various diagnoses and chronic conditions to enhance their community health assessment.  The deidentified information, which covered only patients in the surrounding zip codes, will aid the department in finalizing their assessment.  Additionally, this data may support the development of key interventions or policy changes aimed at enhancing the overall health of the community.

Assistance with Ambulance Service Review of Quality of Care

This request came from an ambulance service seeking a method to review their care quality by comparing it with the treatment provided by the receiving facility after transport.  They were given the diagnoses and procedures of the receiving facility to assess whether the diagnosis and care they provided before transport matched the receiving facility’s treatment.  Analyzing this alignment will help identify potential training opportunities for the ambulance staff and evaluate their overall performance.
