Rural Health Transformation Project

Today is the submission for Year One application deadline for the Digital Health Modernization and Technology Grant Program under South Dakota’s Rural Health Transformation funding initiative!

If you are planning to apply, now is the time to finalize and submit your application. Year One funding closes on June 30, 2026 at 5:00 PM CT, and we want to ensure every eligible organization has the opportunity to be considered.

We also want to share an important reminder: We anticipate requesting additional funding for Grant Year Two which will depend on CMS approval, continuing our commitment to strengthening South Dakota’s healthcare infrastructure and expanding access to modern digital health tools across the state.

What is this Grant All About?

This program is designed to bridge the gap between today’s challenges and tomorrow’s solutions. We are investing in healthcare providers across South Dakota to:

  • Upgrade Legacy Systems: Say goodbye to outdated software and hello to seamless, modern interfaces.
  • Enhance Interoperability: Ensure that patient data moves securely between clinics, hospitals, and specialists.
  • Expand Telehealth Capabilities: Bringing world-class care to the most rural corners of South Dakota.
  • Boost Cybersecurity: Protecting patient privacy with the gold standard of digital defense.

Who Should Apply?

Federally Qualified Health Centers, Critical Access Hospitals, Tribally operated clinics, Community-Based Mental Health Centers, Long Term Post-Acute Care Facilities, other South Dakota healthcare services providers or organizations and more. If you have a vision for how technology can improve patient outcomes in South Dakota, we want to help you achieve it.
