South Dakota Health link is pleased to partner with the Community Health Worker Collaborative of South Dakota (CHWSD) for the 12th Annual Fall Forum to bring together technology, clinical practice, and community-level partners.

We are excited to offer exhibitor/education booths and sponsorships as part of our forum. The Fall Forum is attended by more than 250 healthcare professionals from organizations and providers across the state and region, and includes attendees from hospitals, ambulatory clinics, community pharmacists, public health, social services, correctional health, behavioral health, along with other health care specialties and more!

• 7:00 am – 8:00 am CT – Setup
• 8:00 am – 9:00 am – Open
• 10:30 am – 11:00 pm – Open
• 12:00 pm – 1:00 pm – Open
• 3:00 pm – 3:30 pm – Open
• 3:30 pm – Teardown
• 4:45 pm – 5:00 – Vendor Prize Drawing
